About You:

You are a board-certified or board-eligible Pediatric Dentist (Pedodontist).
You have a Dental Doctoral degree from an accredited dental school.
You hold a valid state dental license and Pedodontic certification.
You possess an Oral Sedation Permit (or are willing to obtain one).
