Requirements:
D.D.S, D.M.D, or equivalent Degree from an ADA accredited dental school
Current dental license in the State of Wisconsin
Current registration with the Drug Enforcement Agency
Current CPR certification (BLS â Basic Life Support) for Healthcare Providers through American Heart or Red Cross
